Recreation Emergency Forms (COVID Risk Form) & Restart Plans



Emergency Forms:

Children Programs:

Children attending a full day program, must bring a completed RDN Recreation Emergency form (includes COVID Risk form). Parents, please complete and return this form to the program leader on the first session. Please be aware that the RDN may decline your child’s admission to the program if this form is not fully completed. These forms will be used throughout the program and then destroyed. In future seasons, you may be asked to fill out a new form so that we have the most up to date information for your child. Please note: If your child is required to take medication during program hours, you need to complete an additional Medication Consent Form. You may request this form from either Oceanside Place or Ravensong Aquatic Centre or the Program Leader.

COVID-19 Assumption of Risk Form - this form must be completed regardless of the length of program. It will be attached to your child's account and will not need to be completed again for future programs (except full day programs).

Adult Programs:

Adults attending RDN programs are required to complete the COVID-19 Assumption of Risk Form. Once signed, the completed form will be kept on account for future registrations.
